PNG NID FORM
This form is designed to be prefilled electronically, but you can always print.
Requirements for NID
You have to know the village clan, tribe etc of your father, mother and yourself before filling in this form.
You will be asked to give your finger print for this purpose - which means your finger print is now recorded in the government database (hopefully its a good one) and stored forever so don't get into trouble after you sign up as a Papua New Guinean Citizen.
Oh and don't forget to bring your ID Cards to be identified. Other requirements, please seek these information at the NID Office because they "Make it Happen"!!
PNG PASSPORT FORM
Requirements for New PNG Passport
2 x recent passport sized photos
(take decent photos - no 'rasta'/smile/cap/earings/glasses)
Certified Copy of your Birth Certificate and National Identity Card (get a commissioner of oaths/Lawyers to sign and stamp these copies. Usually free at any district court house)
Complete the passport form (click the image on the left to download) and lodge it at the Immigration and Citizenship Authority Office counter (Koitachi Haus near SP Brewery at Gordons Port Moresby).
If you live outside Port Moresby, you have to complete an Authorization Form to permit a third-part to lodge on your behalf or you can mail your documents to:

K100.00 for the normal service (3 Weeks turn-around time)
K300.00 for the fast track service fee (3 days to 1 week turn-around time)

For Passport Renewal, the same requirements above apply.
They have since separated the functions of the ICA offices so if you are doing passports, visit the Koitachi Haus branch near SP Brewery at Gordons. Waigani Office (Central Govt Office Building is reserved for VIPs or Visas only)
ICA office at Waigani opens at 9AM (or 10AM PNG Time) and closes at 12PM sharp (on the dot) so be sure to get there early (its always packed)
Note: If you go in at 10AM, there is a good chance you will lodge your documents around 2PM or 4PM so go early!

If you are considering applying for a foreigners visa, ICA has a limited online service via https://evisa.ica.gov.pg/evisa
Sign up for this online service and lodge or check the status of your applications online.
PNG STATUTORY DECLARATION FORM
Here's another document that you might want to type information onto instead of hand writing it.
All stat decs are signed at the court house for free. If you are near one, don't waste your money paying someone to sign it for you.
Don't forget to bring a valid ID with you because you will be asked to present one.
If you are filling a stat dec for a car sale or purchase, please bring along original car registration certificate and both seller and buyer must be present to have both stat decs signed (2x stat decs are required by MVIL)
Note that the date on this form is set by default to 2023 (very old) you can always cross it out to ask me for a cleaner copy. (Updated 25/08/2023)
